EverTrue & the GUIDE Model
What is EverTrue?
EverTrue is a nonprofit, faith-centered organization with a long history (formerly known as Lutheran Senior Services). They provide supportive services, senior living, and community-based care to older adults.
EverTrue has been selected by the Centers for Medicare & Medicaid Services (CMS) to implement the GUIDE Model — a new dementia-care program designed for people on Medicare living with dementia.
What the GUIDE Model Through EverTrue Offers
Families who qualify can receive comprehensive support, including:
A dedicated dementia care guide — one point of contact for help navigating dementia care, appointments, and services.
Personalized dementia care plans tailored to needs, safety, and quality of life.
Coordination of home- and community-based services: home care, memory support, supportive housing if needed.
24/7 access to care support and help line — for crises, urgent questions, or support whenever needed.
Respite support and resources for caregivers to reduce burnout and improve sustainability of home care.

How to Access GUIDE Through EverTrue
EverTrue is participating in the GUIDE program for people on Medicare with dementia.
Families or caregivers can contact EverTrue to check eligibility and enroll.
Once enrolled, a dementia care guide will be assigned, and services tailored according to needs and preferences.

EverTrue PACE (Program of All Inclusive Care for the Elderly)
What is PACE
PACE is a comprehensive program that helps older adults with medical or functional needs continue living safely at home rather than moving to a nursing facility. It wraps medical care, therapies, social activities, home care, and transportation into one coordinated system.
EverTrue PACE serves older adults in parts of St. Louis City and County who qualify for the program.
Who qualifies
To enroll in EverTrue PACE a person must:
Be age 55 or older
Live in EverTrue’s service area
Meet criteria for nursing home level of care
Be able to live safely at home with support
What EverTrue PACE provides
Primary care and specialist coordination
Medication management and prescriptions
Physical therapy, occupational therapy, and rehab
Social work and mental health support
Personal care in the home
Meals and nutrition support
Adult day services and social activities
Transportation to the PACE center and medical appointments
A full multidisciplinary care team
Care planning and ongoing monitoring
Support for caregivers
Because everything is coordinated through PACE, families do not have to arrange separate appointments or services.
